To register for HCI Online Modules and Testing, you must download all files provided below. Please read each document carefully and fill out each form as completely as possible. Each document contains instructions for mailing in your completed documents and your payment.

To access the modules, you must enroll first. To enroll, download the registration form below, fill it out and send it in with your registration fee.

After you have completed the Online Modules and Testing program, please print and complete the Exit Interview Form, then mail or fax to:

Upon receipt of this document and verification of your test scores, Housing Connections Inc. will mail you the Certificate of Completion for the course.
